How to create connections between a "homemade" block and another block

Dear all,
my name is Marco and I started to play with OM less then 3 weeks ago. Well, I would like to create several specifics block for cosimulation, but I need to understand how to make connections between a generic block and another generic block.

For example, in following image I implemented a simple "full bridge" rectifier

What I would like to do is to create a connection (like a port) similar to next image

In this way I can use my block ("full bridge" rectifier) in other projects and connect it to generic electrical network or simple electrical component like resistors or lowpass filters.

Someone can help me?
