http://pedump.me would be helpful to see the difference between these two DLLs.
I suspect simulink links against a different msvcrt.dll (or something alike) than the one libgfortran-3.dll was compiled with in mingw. (dll hell deja-vu?)

I think there is a dependency issue in OpenModelica-exported fmus. Could you try replacing the libgfortran-3.dll (zipped in the FMU) with this one:
